Client Service Agreement Overview

- Frances Touch Consulting LLC provides non-medical senior transition consulting and care navigation services.
- Services include assessment, education, facility coordination, Medicaid and Medicaid-pending guidance, and transition planning.
- Facilities accepting Medicaid do not pay referral or placement fees; therefore, all services are client-paid.
- Fees are due prior to service initiation unless otherwise agreed in writing.
- Consultant does not provide medical or legal advice and does not guarantee placement.
Compensation & Payment
Client understands and agrees that:
- Facilities accepting Medicaid do not pay referral or placement fees
- Consultant’s services are compensated directly by the Client
- Fees are for professional consulting, coordination, and advocacy services
Fee Structure - Private Pay (select applicable, cost will be discuss at consultation):
- ☐ Standard Transition Consulting Fee
- ☐ Urgent Discharge Coordination
- ☐ Medicaid-Pending Support
- ☐ Ongoing Advocacy (if applicable)
Payment is due prior to service initiation unless otherwise agreed in writing.
Disclosure
Consultant does not receive compensation from Medicaid-funded facilities for referrals. If a private-pay placement occurs, Consultant may receive a referral fee from the facility, which will be disclosed to the Client.
Termination
Either party may terminate this Agreement in writing. Fees for services rendered are non-refundable.
How Frances Touch Consulting Is Paid
Frances Touch Consulting operates as a client-advocate based senior transition service.
Important to Know:
- Medicaid and Medicaid-pending facilities do not pay referral or placement fees
- State and federal regulations prohibit compensation tied to Medicaid placements
- To remain ethical, compliant, and unbiased, our services are paid for by the client or family
